Fresh Start Support
Fresh Start Supportive Living LLC is a Georgia based supportive housing program that provides safe, structured, and dignified transitional housing for veterans, individuals returning from incarceration, and community members facing homelessness or housing instability. Our mission is to create a stable foundation where residents receive housing, accountability, life-skills, and coordinated support services that promote long term independence.

Care Planning Session
The Care Planning Session is a collaborative, resident-centered meeting designed to assess needs, identify goals, and develop an individualized plan that supports stability, independence, and successful transition to permanent housing.
The session establishes a clear goal and connects residents to appropriate services and resources.

Safety Assessment
The Safety Assessment is a structured evaluation conducted to identify potential risks to residents, staff, and the housing environment. Its purpose is to ensure a safe, supportive living space while proactively addressing concerns that may impact stability, well-being, or successful program participation.
The Safety Assessment aims to support trauma-informed care planning & ensure compliance with program policies and regulatory standards

Housing Consultation
The Housing Consultation is a structured, individualized meeting designed to support residents in identifying, securing, and maintaining stable permanent housing. This service focuses on housing readiness, barriers to placement, and the development of a clear transition plan from transitional to independent living.
The Housing Consultation aims to educate residents on housing options and tenant responsibilities & develop a realistic and achievable housing transition plan.
